Anaphylaxis treatment

Anaphylaxis:

• Epinephrine i.m 0.01mg/kg/dose of 1:1000 dilution( 1mg/ml)
• If no response - 2 more dose (total upto 3) are given with gap of 5-10mins
• If no response - I.V glucagon loading dose 20-30mcg/kg (max: 1mg) over 5 mins - maintenance 5-15mcg/kg/min

 Respiratory distress

Stridor:

• Nebulise Epinephrine - 0.5ml/kg
• If no response - 2 more nebulizations(total 3) can be done with gap of 5-10mins

Wheeze:

• Nebulise Salbutamol -  0.15mg/kg
• If no response - 2 more nebulizations( total 3) can be done with gap of 5-10mins

Hypotension 

• 20ml/kg NS bolus 
• If no response - 2nd bolus 20ml/kg NS after 5-10mins
• If no response - Infusion Iv Epinephrine - start @ 0.05mcg/kg/min - titrate by 0.02mcg/kg/min upto effect
• If no response - Infusion Iv NorEpinephrine - start @ 0.05mcg/kg/min - titrate by 0.02mcg/kg/min upto effect (max: 2mcg/kg/min)
